In the case of the Kaypro 10, it is defined in the EPROM. Others, I
suspect, store it in the BIOS.

Rather than a driver, you would need to patch the BIOS.

I am familiar with only "The Programmers CP/M Handbook", but I would
guess that each will supplement areas that are weak in one or more of the
others.
