Need help identifying the following IC/CHIP - The next saga

Hi,

Thanks for everyone who helped identify this EPROM chip. I just got yet
another BBK-S4 that works. What I want to do is get a replacement EPROM
so that I can fix the first board. Anyone know where I can get this
chip from???
