Need help identifying the following IC/CHIP

Well...

Looks like an EPROM. I imagine there was a glass window on top of the
chip...More or less the equivalent of floppy disk on a chip. Copied with
an EPROM programmer. Good luck in finding someone that can copy an
identical one for you...
