In the US, the headlights can not be turned off.

That's because they are on the accessory circuit. It's a non-essential
circuit that gets shut off during cranking. Interestingly, there are
sometimes two circuits for the essential devices (like the fuel pump), one
run and one start. Failure of the run circuit but not the atart circuit is
not a uncommon problem. Sometimes the key can be backed off slowly until
the starter disengeges but the ignition doesn't switch from the start
circuit to the run circuit.

That's because of a) your truck/SUV is just Too Durned Big, or b) your
headlights are focused too high for city life. I'd imaging a, but I drive a
truck at work that is both a and b. :)
