I do indeed have the full documentation set for the 28.8 version of that
modem, including the quick reference sheet, which appears to be what you
want. I'll see about scanning it in the next couple days, as I need to set
up my large format scanner. The quick reference sheet is larger than the
8.5x11 scanner I currently have set up, and it didn't turn out too well
when I tried to stitch the images together.

Strangely enough, this modem appears to be barely on topic.
