That certainly sounds like a custom board. Everex did often use custom
boards, even in some of their AT class systems.

IIRC, Everex has been more or less out of business for quite some time.
For some reason, 1997-1998 seems to come to mind. Have you tried an
internic whois on the domain to find contact info?

I have a number of Everex systems and parts stored away currently. I
remember having difficulties finding documentation and drivers for certain
items years ago.
