Please Help Identify Mystery ISA Card

I missed the first original post of this, but it's definately a SCSI control
chip. iin fact I have one of these chips on a card somewhere.

it is also used in amiga's etc for their scsi I/F,. I would assume that if
you are using it under a windows OS, it should pick it up. I know it used to
with mine. it's been a while since i've used it tho.
