6X64SYPC100 RAM module

I am trying to indentify a ram module for replacement.

It is a 168-pin DIMM.

I has a total of 16 chips on it (8 in front, 8 in back)
There is a stick-on tag that reads:

16X64SYPC100 882010
       B A R C O D E
Warranty VOID if Removed
12/99 0773 00011

On the back (center) if reads:

M500PCG REV A
PCSDRAM REV 1.0

I don't see a name anywhere.
