The Therac-25 (and several other poorly-made machines) are in a book
called "Fatal Defect: Chasing Killer Computer Bugs" by Ivars Peterson,
ISBN 0-679-74027-9... it's a bit dry at times, but has some good stories
of programmers that I'm glad I'm not :)
