Don't blame you for not claiming the HW that was used in that scan!

Do you have the case?!?! Knowing the info off of the case would really
help in identifying this. Are you sure it's a VAX board? based on what
little I can see of it, it could have come out of a VAXstation, DECstation,
or even a early Alpha such as a DEC 3000. The SIMM slots are an important
fact, that means it's not one of the earlier VS3100's.

What ports and how many?

If you don't have the RAM, you've a potential problem. You don't want to
know what RAM costs, these aren't normal PC SIMMS, they aren't even
abnormal PC SIMMS.

BTW, does anyone know where I can get at least 8 or 16Mb of RAM for a
VS4000/90 cheap? I just need enough so that I can test the system, since
if I know it worked I'd be willing to buy a decent amount from a dealer.
