I don't know about making a clear sleave since I haven't seen a whole
caddy yet. I bought a drive off of Ebay, and it had a disk still in it,
like yours. I was able to take the top off of the bare drive and pull
the cd out. I think I pulled them out separately, but am not sure on
the order.

I don't have the drive anymore, so I can't look at it to remind myself.
